Trials and cohorts disagree with each other; WHO recommends not using sweeteners for weight control.
Overview
This is the rare case where two kinds of study answer differently, and the platform shows exactly that. In randomised trials, sweeteners produced a reduction in adiposity measures; in prospective cohorts, their intake is associated with increased adiposity and chronic disease risk. Facing that disagreement, the World Health Organization recommends that sweeteners not be used as a means of weight control or of reducing the risk of non-communicable diseases — explicitly labelled a conditional recommendation. To this is added a caveat without which any generalisation would be wrong: sweeteners are metabolised differently from one another, and extending a finding from one to all of them is not warranted.
